First Lieutenant John Thomas "Jack" Wallace 374444670/7562 USMCR was from Detroit Michigan. He was born on April 14, 1945. He arrived in Vietnam on January 12, 1970. He was 26 years old at the time of his death.. He was originally assigned to HMM-364 but was reassigned to HMM-262 when 364 was decommissioned. He died on April 20, 1971 in Quang Nam Province. He was the aircraft commander of a CH-46D ET12 which crashed in about six to ten feet of surf about one kilometer north of Marble Mountain Air Facility due to non-hostile causes. While flying at approximately 600-700 feet at 90-100 knots the rotor blades of the aircraft meshed due to a mechanical failure. His body was recovered. He was not married. His religion was Protestant. His race is officially listed as Caucasian. He had two year’s service at the time of his death. Jack was the last member of HMM-262 to die in Vietnam.
